Informed Consent for Treatment via Telemedicine

This consent explains the independent physician review coordinated through Peak Compounds, its limitations and the information needed to assess your request. Read it before completing the medical questionnaire. Email us if you need clarification before proceeding.

If you have a medical emergency, seek in-person emergency care immediately or contact the emergency services in your country. This service is not an emergency service.

Who Provides the Review

Peak Compounds provides software and coordination for customer intake, physician review and pharmacy fulfilment. We are not a medical provider or a compounding pharmacy, and we do not store, manufacture, dispense or ship medication.

An independent physician reviews the medical information you provide and makes the prescribing decision. Our pharmacy partner prepares and dispatches approved orders.

Mandatory Questionnaire and Independent Decision

• I understand that a medical questionnaire is mandatory with every purchase, and I must provide complete and accurate information.
• I understand that the physician makes the prescribing decision independently and may decline my request. Payment does not guarantee approval.
• I understand that if a prescription is not approved, my payment minus the physician review fee will be refunded. The physician review fee is nonrefundable, including when a prescription is declined.

Scope and Limitations of Remote Review

• I understand that the review uses information I provide electronically rather than a physical examination, and that incomplete information, technical problems or transmission errors may affect or delay the review.
• I understand that the review has a limited scope, is not appropriate for every condition and does not replace ongoing care from my own healthcare provider.
• I understand that I should consult my own physician about my overall health and treatment and seek an in-person assessment when needed.
• I understand that a prescribing decision does not guarantee a particular treatment outcome or establish permission to import medication into my country.

Prescription Copies and Import Requirements

• I understand that the mandatory physician review and any resulting prescription serve the pharmacy’s internal compounding and dispensing process.
• I understand that prescription copies are not supplied to customers and that Peak Compounds cannot provide a prescription for use in clearing a package through customs.
• I understand that this service does not provide a prescription guaranteed to be recognised in my country. I must have my own prescription ready to meet receiving-country requirements and check what documentation the authorities will accept before ordering.
• I understand that international dispatch, including to the US, physician approval, having a prescription and successful previous deliveries do not guarantee import permission or customs clearance.

Information Shared for Review and Fulfilment

I understand that my questionnaire and relevant health information are reviewed electronically by the independent physician and that information needed to compound and dispense an approved order is shared with the pharmacy partner. Order, delivery and payment information is handled by the providers involved in those services. The Privacy Policy explains these categories, purposes, recipients and international processing.

I understand that remote communications and electronic systems have privacy and security risks and that no internet-based system is completely secure. I can email support with questions about how my information is handled.

Withdrawal, Cancellation and Refund Cutoff

I may withdraw my consent to further telehealth review by emailing [email protected]. I understand that withdrawal does not undo work already performed or automatically require deletion of records that must be retained under applicable obligations.

The physician review fee is nonrefundable. The remainder of my order payment can be refunded only before the pharmacy has been paid. Once the pharmacy has been paid, the sale is final and refunds are not offered. If I want to cancel, I must contact support immediately. See the Refunds and Returns Policy.

Delivery and Customs Risks

I understand that customs may refuse entry, confiscate or destroy a shipment and that I bear the full financial loss in those cases, without reimbursement or refund. I am responsible for applicable import charges and timely responses to customs or courier requests. A discretionary future-order discount is not guaranteed.

I understand that delivery estimates are not guarantees and that returned packages are subject to the reshipping terms in the Shipping Policy, including the exception under which Peak Compounds covers reshipping if the original shipment was sent to the wrong address, regardless of how the error occurred.

Patient Acknowledgment

By completing the medical questionnaire and proceeding with the review, I acknowledge that I have read and understood this consent, had the opportunity to ask questions, and voluntarily consent to the independent physician reviewing my information within the scope described above. This notice describes consent; it does not expand the review into ongoing care or guarantee local recognition of a prescription.

Nothing in this consent or the related policies excludes rights, remedies or responsibilities that applicable law does not allow to be excluded.
